Mr PEHM: I think it was still possible to make a finding that there was a failure in
clinical governance in Central Adelaide Local Health Network by failing to do a comprehensive
investigation, whether you call it an RCA or another type of investigation, and produce a report that
clearly set out for everyone involved in the situation what occurred and clear recommendations which
were then given the force, the imprimatur, of the chief executive and so they became registered and
capable of follow-up and implementation.
